Insurance  

We are committed to helping you receive your full insurance benefits; however, you are still personally responsible for your account, and we encourage you to contact us if your policy has not paid within a reasonable timeframe.  

Community Physicians Group files participating insurances as a courtesy. Please verify your provider is in network with your insurance company prior to treatment. Deductibles and co-insurance amounts are collected at the time of service. Community Physicians Group is in compliance with Title VI of the Civil Rights Act. If you do not see your insurance company on the list below, please contact the office of your provider for further assistance.
