C. Joseph Chouteau, M.D.
Dr. C. Joseph Chouteau is a board certified family practice physician who limits his practice to ages 55 and older, with special interests in diabetes, COPD, and heart disease. He also treats skin lesions/skin cancers and does healthy life maintenance.
Dr. Chouteau received his medical degree from the University Of Oklahoma School Of Medicine in 1978, and completed his Family Practice Residency at the University Of Oklahoma, Tulsa Medical College in Tulsa, OK. He started in private practice in Miami, OK in April of 1981 and has been in Grove, OK at the Professional Center since 1987 doing business as Family Care Specialists.
Dr. Chouteau joined the Community Physicians Group January 1, 2012. He holds memberships in the American Academy of Family Practice.
He is a member of the American Academy of Family Practice and has served on several boards including the INTEGRIS Health Corporation board from 1995-2011. He served as Medical Director for Amedysis Home Health from 2006-2010, and currently serves as the Medical Director for Carter Home Health Agency 2010 to present, and Good Shepherd Hospice since 1995. Dr. Chouteau was selected as the INTEGRIS Grove Hospital Physician of the Year in 2008, and served as their Chief of Staff in 1999, 2003, and 2005.
Dr. Chouteau has volunteered at the Grove Christian Medical Clinic since 2001. The clinic provides free care to those in the community who can not afford to see a doctor.
When not providing care or volunteering, he enjoys golf, tennis, scuba diving and is a private pilot.
He and Dana, his wife of 38 years, have two children. Ben and Jennifer have blessed them with 8 grandchildren.

Richard Tidwell, M.D.
Dr. Richard Tidwell is a Family Practice Physician who enjoys seeing patients of all ages. He provides all types of care from well exams, sick exams, obstetrics, small office procedures and colonoscopies to Nursing Home visits.
Dr. Tidwell received his Medical Degree from the University of Oklahoma. He completed his residency at John Peter Smith Hospital in Fort Worth, Texas where he served as Chief Resident his Senior Year. He also was honored as "Resident of the Year" his Senior Year. He has been in practice for 18 years. He is Board Certified as a Diplomat with the American Board of Family Physicians. He is a member of the Oklahoma Academy of Family Physicians, the Oklahoma State Medical Association and the American Academy of Family Physicians. Samuel Ratermann, M.D.
Dr. Ratermann is a board certified family medicine physician who specializes in full spectrum care of the entire family from birth to the end of life. He treats infants, children, and adults including obstetric care for expectant mothers. In addition to routine clinic services including well and sick visits, Dr. Ratermann provides care for patients in the hospital as well as those receiving care at nursing homes in the Jay and Grove areas.
Dr. Ratermann graduated Summa Cum Laude from Oklahoma State University before entering the University of Oklahoma Medical School. He completed residency at Via Christi in Wichita, Kansas where he earned top resident two out of three years. Since graduating from residency he has served as a board member for the Green / Kiamichi District of the Oklahoma Academy of Family Physicians where he currently holds the position of vice president and co-chair of the patient advisory committee. Locally Dr. Ratermann serves as chair of the Intensive Care committee and co-chair of the Information Technology Committee at Integris Grove Hospital.

Zachary T. Bechtol, M.D.
Dr. Zachary Bechtol is a Family Medicine physician who cares for patients from birth thru the golden years. He provides obstetric care including delivery and C-Sections, postpartum care to mothers, and newborn care. He is passionate about preventive medicine for patients in his care. He performs wellness physicals, colonoscopies for colorectal cancer screening, breast cancer screenings and screening for skin cancer. He performs skin biopsies, and localized treatment of precancerous and cancerous skin lesions with Cryotherapy and Lesion removals. Dr. Bechtol also does emergent care for the whole family.
Dr. Bechtol received his medical degree from the University Of Oklahoma School Of Medicine and completed his Family Practice Residency at St. Joseph's Hospital in Wichita, Kansas in 1998. In 2008 he was recognized by the Oklahoma Academy of Family Physicians as "Oklahoma's Family Physician of the Year". Dorothy Mayes, NP
Dorothy Mayes is a Nurse Practitioner with over 30 years of experience in the nursing field. A native of New Mexico, Mayes has lived in Oklahoma for the past 39 years.
She received her Associate degree of Nursing from NEO College in Miami, OK in 1983. She then went on to Pittsburg State University in Kansas where she earned her Bachelor and Master Degrees in Nursing.
INTEGRIS Baptist Regional Center became her professional home where she worked on the surgical floor and the intensive care unit. She also has worked with primary care physicians in Miami in their individual clinics.
Mayes became part of the Community Physicians Group in March of 2013 when she joined the practice of Dr. Zachary Bechtol. "I chose to come to CPG because of their innovative ideas to improve patient care", she explains. "I love helping patients to improve their outcomes and decrease their risks when ill."
She is accredited by the American Academy of Nurse Practitioners of which she is a member.

Angela Fangmeier, M.D.
Dr. Angela Fangmeier is a pediatrician who cares for children newborn through age eighteen. She provides well child exams, immunizations, hearing screening, ADD/ADHD evaluations as well as acute health care.
She received her medical degree from the University of Oklahoma. She continued at the University of Oklahoma to complete her internship and residency. Dr. Fangmeier has practiced medicine for 6 1/2 years; she has been with Community Physicians Group for 6 1/2 years. She is board certified by the American Board of Pediatrics and holds memberships with American Medical Association, American Academy of Pediatrics and has current certifications in BLS (Basic Life Support), PALS (Pediatric Advanced Life Support), and Neonatal Resuscitation. Joseph Aldrich, Jr. D.O., FACOP, FAAP
Joseph Aldrich, Jr. D.O., FACOP, FAAP is board certified in pediatrics. After receiving his Bachelor of Science at Rockhurst College in Kansas City, MO, he went on to the Kansas City College of Osteopathic Medicine and completed his pediatric residency there. Dr. Aldrich will join the Community Physicians Group June 2013.
Dr. Aldrich chose to come to CPG because of the people. "Their honesty and support is rare, and to be treasured. The people at Sager Creek Pediatrics and Family Care are the type I have always wanted to find. I look forward to practicing with Dr. Fangmeier and Latina Shelley, APN, CPNP," he expresses.
He decided to become a pediatrician when he was a junior in high school. "I can't imagine doing anything else. Nothing is more rewarding than helping a child remain well. They are our future. Nothing is more fulfilling than putting a parent's worries to rest," he explains.
Dr. Aldrich was a medical officer in the navy and has taught at medical school achieving the honor of professor of pediatrics. .
Born in Kansas City, MO, he met his wife Diane who is an elementary school teacher, on a blind date. They dated for four years before they married. The couple have four children, all are married: Two are physicians; one is a teacher and the other is a pharmacist and are blessed with eight grandchildren.

Latina Shelley, APN
Latina Shelley is and Advanced Pediatric Nurse. After receiving her BSN nursing degree from the University of Arkansas in 2006, she went on to the University of Missouri, Kansas City where she graduated in 2012 as a Certified Pediatric Nurse Practitioner.
Latina chose to come to the Community Physicians Group (CPG) because she likes the smaller community setting and the opportunity to provide closer patient care.
Being educated in preventing illness and disease is very important to Latina. She enjoys counseling parents on not only how to care for their children, but also why the care helps in achieving desired outcomes.
Always wanting to be in the healthcare field to care for children, she and her husband Brian have 2 children of their own, Wyatt and Magalene. The family enjoys the simple things in life, camping or just watching movies together on the couch.
Ashish Mathur, M.D.
Ashish Mathur, M.D., chose to be a Family Practitioner because he enjoys the diversity of care for all ages and all medical problems.
Dr. Mathur provides medical care for newborns, children, adults, and the elderly in a variety of areas, including wellness exams, acute care, urgent care, minor skin procedures and colposcopy. He also has an interest in Sports Medicine. Dr. Mathur completed his residency at the University of Oklahoma in Tulsa, Oklahoma, where he served as Chief Resident. He completed his residency training in family medicine in June 2007. He received his medical degree from Universidad Iberoamericana in the Dominican Republic and has current memberships with the American Academy of Family Physicians. Dr. Mathur and his wife, Pragya, recently got married in India. Having grown up in Africa, he enjoys Northwest Arkansas' proximity to nature. He has lived in the Midwest for a number of years and is especially drawn to the strong family values that add to the vibrant culture of Gentry. John Caswell, M.D.
John E Caswell, M.D., a Family Practitioner specializes in medical care for newborns, children, adults, and the elderly. Dr. Caswell completed his residency training in family practice in June 2006, and began practicing at the Gentry Medical Center in July 2006.
Dr. Caswell provides care for patients in a variety of areas, including wellness exams, acute care, urgent care, minor skin procedures, colonoscopy, and EGDs. He completed his residency at AHEC Northwest Family Practice Residency Program in Fayetteville, Arkansas. Dr. Caswell received his medical degree from Louisiana State University Health Sciences Center in Shreveport, Louisiana, and has current memberships with the Arkansas State Medical Society and American Academy of Family Physicians. Catherine Casey, APN
Catherine Casey is an Advanced Practice Nurse and board certified Family Nurse Practitioner. She provides medical care for newborns, children, adults and geriatrics in all areas including wellness exams, acute care, chronic care and minor office procedures. She has been in practice for six years and has a special interest in women's health and chronic disease management.
Catherine came to CPG in September 2008 where she joined Dr. John Caswell and Dr. Ashish Mathur at the Gentry Medical Center. She graduated from the University of Arkansas in 1997 with a Bachelor of Nursing degree and worked as a Registered Nurse until obtaining her Master of Nursing Science degree from the University of Arkansas for Medical Sciences in 2002. Prior to joining CPG Catherine worked for Cherokee Nation providing primary care for tribal members.
In her free time Catherine enjoys gardening, cooking and horseback riding. She has lived in Siloam Springs since 1992 with her husband Mark who calls Siloam Springs his home town. They live on the family farm and have three children Autumn, Miles and Emma-Claire.
George Benjamin, M.D.
Dr. George Benjamin is a Family Practitioner caring for adults and children. He provides a variety of services including treadmill testing, vasectomy, circumcision, wellness exams (men, women, and children), minor procedures as well as acute medical care.

Roger Youmans, M.D.

Elizabeth McCord, MSN, MNSc, FNP
After accomplishing her Master of Nursing degree at the University of Arkansas for Medical Sciences, nurse practioner Elizabeth McCord, BSN, MNSc, FNP has returned to her home town of Siloam. She joins the other seven providers at the Siloam Springs Medical Center.
"I love taking care of people of all ages, and a small community such as Siloam Springs is perfect for that, she explains. I chose medicine because I love people, and I love challenge. I chose to come to CPG because of the diversity and variety of people and healthcare issues that it offers."

Carleen Huffman, ARNP-CNP
Mrs. Huffman is an Advanced Registered Nurse Practitioner in Family Practice. She cares for infants, children, young adults and families; she also provides services in women's health care and diabetes care. She is practicing at the Kansas Medical Clinic with Dr. Dan Pummill. She diagnoses, prescribes, and performs minor office procedures. She graduated with her Associates in Nursing in 1992 from Fresno, California. She managed an ICU and Telemetry unit until she moved to this area where she worked in Surgical ICU and ER. Mrs. Huffman graduated from University of Oklahoma in 2004 with her masters becoming an Advanced Registered Nurse Practitioner.

Dan Pummill, M.D.
Dr. Pummill is a Family Practitioner providing medical care for children, adolescents, adults, and geriatrics. He attends to patients in all areas of health care, including wellness exams, acute care, DOT physicals, ENT problems including allergy work ups and endoscopy of the nose and throat, travel physicals, and workers compensation. He has radiology and lab services in the clinic. Dr. Pummill also provides osteoporosis, dementia, incontinence, and E.D. evaluations for patients.
He received his medical degree from the University of Oklahoma. He continued with a residency and internship at the University of Oklahoma Family Practice Clinics. Dr. Pummill has practiced medicine for 22 years and was one of the founding members of Community Physicians Group in 1998. Dr Cheryl Kroeker completed her Undergraduate degree at McPherson College in McPherson, Kansas in the spring of 2000. Cheryl Florence, APRN, NP-C
Cheryl Florence is and Advance Practice Registered Nurse. After receiving her Associate of Science in Nursing in 2007 from the Tulsa Community College, Tulsa, OK, she went on and accomplished her Master of Science in Nursing, Family Nurse Practitioner from the Saint Louis University, St. Louis, MO in 2012.
She joined the Community Physicians Group in February, 2013, because of its stability, trustworthiness, and family-friendly atmosphere.

Previously employed as a middle school science teacher, health promotion and disease prevention were her two passions. She was devoted to teach the students how to take care of their health. Eventually, she realized it was time to make the switch from education to nursing in an effort to help patients define, and ultimately reach their health goals.
Mother of four, Cheryl's main focus in on God and family. She and her husband Greg find themselves at swim lessons, church, school functions, the gym or in their vegetable garden.
Fred West, PA-C
Fred West PA-C practices at Westville Medical Center as a Physician Assistant. He can diagnose, prescribe, and perform procedures. Mitch Collier, M.D.
Mitchell J. Collier, M.D. is a Family Practitioner providing care for newborns, children, adults, and geriatrics. He attends to patients in all areas of health care, including but not limited to wellness exams, acute care, obstetrics (including c-sections) and orthopedics, and has radiology and lab services in the clinic.
Dr. Collier received his medical degree from the University of Oklahoma Medical School where he graduated with honors. He went to OU Family Medicine residency program where he was given the Joseph Salamy Intern of the Year Award. He also was given the Paul E. Tietze Memorial Award for commitment to patient care at the end of his residency. Dr. Collier joined Community Physicians Group in 2004. He is board certified by the American Board of Family Medicine.
Dr. Mitch Collier is married with two children. He enjoys fishing, football, computers, and cooking.
