Angela Fangmeier, M.D.
Dr. Angela Fangmeier is a pediatrician who cares for children newborn through age eighteen. She provides well child exams, immunizations, hearing screening, ADD/ADHD evaluations as well as acute health care.

She received her medical degree from the University of Oklahoma. She continued at the University of Oklahoma to complete her internship and residency. Dr. Fangmeier has practiced medicine for 6-½ years; she has been with Community Physicians Group for 6 years. She is board certified by the American Board of Pediatrics and holds memberships with American Medical Association, American Academy of Pediatrics and has current certifications in BLS (Basic Life Support), PALS (Pediatric Advanced Life Support), and Neonatal Resuscitation. She served as Chief of Staff in 2005 for Siloam Springs Memorial Hospital.

Dr. Fangmeier is the mother of two and enjoys reading, bird watching and collecting corvettes with her husband.

Yvonne Couch, ANP, CPNP
Yvonne Couch is a certified pediatric nurse practitioner providing care for children newborn up to age 18. She provides well child examinations, sports physicals, acute health care, and adolescent gynecological care.

In 1994, Couch earned her B.S. in Microbiology from the University of Arkansas. She worked as a food microbiologist for eight years with Consumer Testing Laboratories before seeking her master’s degree. In 2006, Couch graduated from Yale School of Nursing in New Haven, CT with a Master's in Pediatric Nursing as a Pediatric Nurse Practitioner.

Couch grew up East of Highfill, Arkansas on a family farm, graduating in 1989 from Gentry High School. After receiving her master's in pediatric nursing, she and her husband returned to Northwest Arkansas to be closer to family and friends.

Some of Yvonne's leisure activities include hiking (especially in the Buffalo River Valley), reading, and identifying and photographing wildflowers.

Rebecca Ramick, M.D.
Dr. Ramick is a successful Pediatrician offering the benefits of clinical expertise in the field of general pediatrics, specialization in the field of Pervasive Development Disorders, Autism, and other disorders of development, and excellent care-giving skills. She likes to practice general pediatrics in a patient-friendly environment with special emphasis on patients with Pervasive Development Disorders, Autism, and other disorders of development. Dr. Ramick received M.D. from University of Arkansas for Medical Sciences, School of Medicine, Little Rock, AR, and did her residency and internship at Arkansas Children’s Hospital, Little Rock, AR. She is Board Certified American Academy of Pediatrics.

Dr. Ramick’s experience includes assistance in growth of practice from two to six partners over a ten year period and management of high-volume pediatric practice. She performs circumcisions routinely with plastibell under local anesthesia. She has special interest in the area of Pervasive Development Disorders, Autism, and all areas of development delay.

Ashish Mathur, M.D.
Ashish Mathur, M.D., chose to be a Family Practitioner because he enjoys the diversity of care for all ages and all medical problems.

Dr. Mathur provides medical care for newborns, children, adults, and the elderly in a variety of areas, including wellness exams, acute care, urgent care, minor skin procedures and colposcopy. He also has an interest in Sports Medicine. Dr. Mathur completed his residency at the University of Oklahoma in Tulsa, Oklahoma, where he served as Chief Resident. He completed his residency training in family medicine in June 2007. He received his medical degree from Universidad Iberoamericana in the Dominican Republic and has current memberships with the American Academy of Family Physicians. He is certified in BLS (Basic Life Support), ACLS (Advance Cardiac Life Support), and PALS (Pediatric Advanced Life Support).

Dr. Mathur is also fluent in Spanish, among other languages.

Dr. Mathur and his wife, Pragya, recently got married in India. Having grown up in Africa, he enjoys Northwest Arkansas’ proximity to nature. He has lived in the Midwest for a number of years and is especially drawn to the strong family values that add to the vibrant culture of Gentry. His hobbies include biking, hiking, boating, gardening, and cooking.

John Caswell, M.D.
John E Caswell, M.D., a Family Practitioner specializes in medical care for newborns, children, adults, and the elderly. Dr. Caswell completed his residency training in family practice in June 2006, and began practicing at the Gentry Medical Center in July 2006.

Dr. Caswell provides care for patients in a variety of areas, including wellness exams, acute care, urgent care, minor skin procedures, colonoscopy, and EGDs. He completed his residency at AHEC Northwest Family Practice Residency Program in Fayetteville, Arkansas. Dr. Caswell received his medical degree from Louisiana State University Health Sciences Center in Shreveport, Louisiana, and has current memberships with the Arkansas State Medical Society and American Academy of Family Physicians. He is also certified in BLS (Basic Life Support), ACLS (Advance Cardiac Life Support), PALS (Pediatric Advanced Life Support) and NALS (Neonatal Advanced Life Support).

Dr. Caswell is married to Becky, a 9th grade biology teacher they have two daughters Kimberly and Kaitlyn. He enjoys the Northwest Arkansas area and especially the strong family commitment of Gentry. His hobbies include water skiing, hiking, and camping.

Catherine Casey, APN
Catherine Casey is an Advanced Practice Nurse and board certified Family Nurse Practitioner. She provides medical care for newborns, children, adults and geriatrics in all areas including wellness exams, acute care, chronic care and minor office procedures. She has been in practice for six years and has a special interest in women's health and chronic disease management.

Catherine came to CPG in September 2008 where she joined Dr. John Caswell and Dr. Ashish Mathur at the Gentry Medical Center. She graduated from the University of Arkansas in 1997 with a Bachelor of Nursing degree and worked as a Registered Nurse until obtaining her Master of Nursing Science degree from the University of Arkansas for Medical Sciences in 2002. Prior to joining CPG Catherine worked for Cherokee Nation providing primary care for tribal members.

In her free time Catherine enjoys gardening, cooking and horseback riding. She has lived in Siloam Springs since 1992 with her husband Mark who calls Siloam Springs his home town. They live on the family farm and have three children Autumn, Miles and Emma-Claire.

George Benjamin, M.D.
Dr. George Benjamin is a Family Practitioner caring for adults and children. He provides a variety of services including treadmill testing, vasectomy, circumcision, wellness exams (men, women, and children), minor procedures as well as acute medical care.

He graduated medical school from the University of Arkansas in Little Rock, Arkansas. He served two years in the Indian Health Service in Philadelphia, Mississippi. He continued his career with an internship at Harborview Medical Center in Seattle Washington and completed his residency in Family Medicine at the University of Oregon Health Science in Portland, Oregon. He is a member in the American Academy of Family Practice. Dr. Benjamin is board certified by the American Board of Family Medicine and has a Certification of Added Qualification in Geriatric Medicine. Dr. Benjamin has practiced medicine for 35 years and is one of the founding members of Community Physician Group in 1998.

Dale Clemens, M.D.
Dr. Clemens is a Family Practitioner providing medical care for infants, children, adolescents, adults, and geriatrics. He provides a variety of services including cardiac treadmill testing, vasectomy, circumcision, wellness exams (men, women, and children), minor procedures as well as acute medical care. Dr. Clemens is also a Certified Medical Review Officer meeting the local industries and companys changing needs in a drug testing environment.

He graduated medical school from the University of Arkansas. He completed his residency at the Area Health Education Centers-Northwest in Fayetteville AR. He is certified by the American Board of Family Medicine and the American Association of Medical Review Officers. Dr. Clemens has practiced medicine for 24 years and was one of the founding members of Community Physicians Group in 1998.

Carl Duncan, M.D.
Dr. Duncan chose to specialize in Family Medicine because he enjoys providing a full range of health care services spanning from infancy thru adulthood, with a particular emphasis on promoting a healthy lifestyle for all ages. Dr. Duncan provides services such as wellness exams (children & adults), acute care, weight loss counseling and treatment (including childhood obesity), smoking cessation counseling, and the comprehensive management of chronic illnesses. He also performs a variety of procedures such as colposcopy, cryotherapy, circumcisions and other minor procedures.

Dr. Duncan completed his undergraduate studies at Oklahoma Christian University in Edmond, OK. He received his medical degree from Oklahoma University College of Medicine in Oklahoma City, OK and completed his residency and internship at the Great Plains Family Practice Residency Program in Oklahoma City, OK where he served as Chief Resident. Dr. Duncan is board certified by the American Board of Family Practice and holds memberships with the American Academy of Family Physicians and the American Medical Association. Kelly Milburn
Kelly J. Milburn APN is a Fayetteville native that completed her Bachelors of Science in Nursing from the University of Arkansas in 2004. After graduating she moved to Kansas City, MO where she worked on a womens medical-surgical unit as a Registered Nurse for 3 years. While in Kansas City she attended the University of Missouri Kansas-City for her Family Nurse Practitioner. In 2007 she graduated from UMKC and moved back to Northwest Arkansas. She is now board certified Family Nurse Practitioner.

As a Family Nurse Practitioner, Kelly can see patients of all ages and is able to do physical exams, order test, diagnose problems, write prescriptions, and more.

In her spare time Kelly supports the razorbacks and enjoys all the outdoor amenities that this area has to offer. She loves camping, water-skiing, canoing, biking, kayaking, and hiking.

Dan Springer, M.D.
Dan Springer, MD joined the Siloam Springs Medical Center in 1987. His practice includes providing care for ages birth though adulthood. He attends to patients in all areas of health care, including wellness exams, acute care and minor procedures.

Dr. Springer completed his internship and residency at the University of Oklahoma. The American Board of Family Medicine certifies him. Dr. Springer enjoys a variety of activities with his wife and their four children.

Scott Stinnett, M.D.

He received his medical degree from University of Arkansas in Little Rock. He completed his residency at Oklahoma University-Tulsa Medical College. Roger Youmans, M.D.
Dr. Youmans is a Family Practitioner providing medical care for newborns, children, adults and geriatrics. He provides a variety of services for his patients. Carleen Huffman, ARNP
Mrs. Huffman is an Advanced Registered Nurse Practitioner in Family Practice. She cares for infants, children, young adults and families; she also provides services in women’s health care and diabetes care. She is practicing at the Kansas Medical Clinic with Dr. Dan Pummill. She diagnoses, prescribes, and performs minor office procedures. She graduated with her Associates in Nursing in 1992 from Fresno, California. She managed an ICU and Telemetry unit until she moved to this area where she worked in Surgical ICU and ER. Mrs. Huffman graduated from University of Oklahoma in 2004 with her masters becoming an Advanced Registered Nurse Practitioner.

Dan Pummill, M.D.
Dr. Pummill is a Family Practitioner providing medical care for children, adolescents, adults, and geriatrics. He attends to patients in all areas of health care, including wellness exams, acute care, DOT physicals, ENT problems including allergy work ups and endoscopy of the nose and throat, travel physicals, and workers compensation. He has radiology and lab services in the clinic. Dr. Pummill also provides osteoporosis, dementia, incontinence, and E.D. evaluations for patients.

He received his medical degree from the University of Oklahoma. He continued with a residency and internship at the University of Oklahoma Family Practice Clinics. Dr. Pummill has practiced medicine for 22 years and was one of the founding members of Community Physicians Group in 1998. He is board certified by the American Board of Family Medicine and is a clinical Professor of Family Medicine at the University of Oklahoma College of Medicine. His interests include medical instruction of interested high school students, nurses, medical students, and residents. In 2004, Dr. Pummill was honored by the Oklahoma Academy of Family Practice as Physician of the Year.

Dr. Pummill enjoys running, biking, gardening, tennis, scuba, and skiing. He has served on the board of Cookson Hills Christian Ministries since 1979.

Fred West, PA-C
Fred West PA-C practices at Westville Medical Center as a Physician Assistant. He can diagnose, prescribe, and perform procedures. Fred has practiced at Locust Grove Family Physicians, Kansas Medical Clinic, and Siloam Springs Medial Center.
